An applicant for approval of a supervised professional experience in Connecticut must have the following documents submitted directly from the source to this office:
A notarized application and fee of $200.00, in the form of a bank check or money order, payable to "Treasurer, State of Connecticut”;

For purposes of determining eligibility for a supervised professional experience, this office will accept a letter from the applicant's educational program director, on university letterhead, specifying that all academic and fieldwork requirements for graduation have been completed and the date on which the degree will be conferred. A final, official transcript verifying completion of the degree program is required before a license will be issued.
Upon receipt of the application and transcript or program director letter, the Department will notify the applicant in writing regarding the applicant's eligibility for SPE and will provide the applicant with an SPE Approval Form that must be completed and returned directly to this office by the supervisor. The required form may be obtained by selecting this link. Please note that a plan will not be approved prior to receipt of an application and appropriate documentation of completion of education.
Following the completion of the SPE, a report form must be completed by the SPE supervisor evaluating the applicant's performance and be submitted directly to this office. An applicant must receive an evaluation satisfactory to this Department in order to become licensed.
