Most licensees can expect to receive renewal notification approximately 60 days prior to expiration and if not renewed, a final notice 30 days following expiration, at the licensee's address of record on file in this office. Please keep your address current.
Physicians, dentists, nurses, master level social workers, genetic counselors and master's level social workers will receive renewal notifications by email only.
Licensees who have not received renewal notification within these time frames should email this office and request that a renewal application be mailed.
Most licenses expire in the first birth month following issuance and are renewable annually thereafter. Please select this link for professions that expire biennially.
Online renewal is required for Physicians, dentists, nurses, master level social workers, genetic counselors and tattoo technicians. Please select the following link for more information. The Department is currently working on adding this functionality for all other licensed healthcare providers.
Renewal Fee Waiver - Licensees on Active Duty in U.S. Armed Forces
Licensees who are on active in the Armed Forces of the United States may request exemption from the license renewal fee provided they are also a legal resident of Connecticut.
In order to qualify for exemption, the following must be submitted along with the completed and signed renewal application:
A letter from the commanding officer attesting that the licensee is currently on active duty; and
Proof of legal Connecticut residence in the form of a copy of a recent voter registration card or a letter from the town of residence attesting that the licensee is a resident.
You may submit these items directly to the office via email to email@example.com or you may send them via mail to:
Department of Public Health
Renewal Fee Waiver-Active Duty Armed Forces
410 Capitol Avenue, MS #12MQA
Hartford, CT 06134-0308
Please note that fee waiver must be requested each time the license is renewed. If you do not meet the criteria above, you are required to pay the renewal fee.
There is no inactive status for licenses; a licensee would either need to keep the license current or let it lapse due to non-renewal. Once a license has lapsed due to non-renewal and the licensee subsequently would like to reactivate the license, the licensee would need to apply for and meet the requirements for reinstatement specific to the profession.
Pursuant to Section 19a-88(e)(4)f, Connecticut General Statutes, all licenses issued by the PLIS have a grace period of 90 days following expiration during which time the licensee may continue to practice and renew the license. On the 91st day, the license becomes void and the licensee must apply for reinstatement.
